New Student Health Declaration Process

Liverpool Hope University is required by the professional bodies governing particular regulated courses to assess the physical and mental fitness of those entering certain career pathways.

This student health declaration/screening process will provide the University and its appointed Occupational Health Service with information about your health status and fitness for a career in a regulated profession. The process also enables the University to identify whether you are likely to require any adjustments in provision enabled to assist you during your training and/or subsequent employment.

Depending on the information you provide in the questionnaire, you may be contacted by the University's appointed Occupational Health Service, Aintree NHS University Hospital Occupational Health and Wellbeing Service, to provide further information. This could involve you receiving a request to complete a slightly more detailed health questionnaire, telephone contact from an Occupational Health practitioner, or in limited instances, a consultation with an Occupational Health Nurse/Doctor may be necessary.

In exceptional cases, it may be necessary for applicants to obtain a medical report from their GP/medical practitioner, or provide consent where access to GP records is required. Please be advised that applicants are responsible for meeting any costs associated with obtaining medical reports/records as part of the health screening process.

Please ensure that the contact telephone number and email address on the following screen are accurate, as Aintree NHS University Hospital Occupational Health and Wellbeing Service will use those details should they need to contact you.

Withholding or providing false or misleading information when completing the health screening process may result in the withdrawal of a place offer, suspension of studies and/or disciplinary action, which could result in the termination of studies.

Your completed questionnaire should be submitted as soon as possible, as failure to do so will delay your health screening and consequently your admission onto your chosen programme of study.
